PSU Dead I think. Would Love Some Help. Thank you!

Not looking good. Just bought a really nice 450 dollar corsair 1200 wat power supply. The simplest overclocks I do to my new saphire r9 290x card, my monitor will flicker and then the monitor will go black causing me to hard reset my PC. One time when I opened the program GPU-Z the computer hard locked and the reset button didn't work when it hard locked. The computer shutdown instead and then turned on again a few seconds later. I was reading around on websites that your GPU-Z should be listing around 12v or something constant. I am not very smart when it comes to computers, but I don't know why all my listings in GPU-Z are low. Please take a look at this safe link.

Someone said it has to be at a stable 12v or something. My readings are always below that. Is that a PSU problem?

I just bought my new GPU and I am worried I have damaged it. I think it should be okay. I think my PC runs fine when my card is on stock.
