Hi! I've been having this issue lately and I can temporarily fix it by pressing the reset button on my computer. I've looked around and I see that it could be a loose cable, but I don't know how to check without the fear of breaking something. My SSD is an Agility something 4 (I was a pretty big noob when I ordered the computer from ibuypower lol). I did see a cord that was loose though, : http://i.imgur.com/gTGS8Wa.jpg Is that the problem?

Solution
in your photo on the back of the cd rom drive the small cable is the sata data cable the larger is the power cable. the sata data cable have a clip most time. if the sata cable falls off most times it because the port snapped from stress (wire) when built. your error sounds like it could be two issue. one could be a firmware bug where the drive will die after so much time running. (some drives had this firmware bug). check with the ssd vendor tool that the ssd has the newest firmware also run ssd fitness test from the vendor or hdtool. right now I back any of your data before the drive dies and if it under warranty start an rma.

What do you mean by SSD vendor tool? Can you link it?
