unable to format memory card

I have 16gb class 10 memory card when i try to format my system says unable to format sd card when i try to format in cmd it says successfully formated when i insert in my phone all the format data will come again please help on this issue

Have you changed the format type of the memory card? I imply is it change FAT32 to NTFS or not, if it's possible, then changes the type and reformat the memory card. See what happened after that.
