Display loses signal with computer, and Computer essentially "crashes" when gaming

This has happened to me 3 times today. I played a few games like League of Legends, or watched some YouTube videos, and then my display would go blank but my computer would still be running fine. I've tried disconnecting and reconnecting the HDMI, but nothing happened. I even tried a DVI-D, and a VGA connector as well but still, with no luck.

Just a few minutes ago, my display had gone blank but came back and there was no sound. The computer also acted very sluggishly, but the CPU was at 33 degrees Celsius, ~%20 utilization. Everything else was lower than that. I had to reboot. Now it's working smoothly like before.

Does anybody know a fix to this? I'm assuming I need to either get another RAM stick for my single-channel I'm running now, a new hard drive, or video card. Heck, maybe even a new motherboard, but hopefully not.

CPU: AMD Athlon x4 760k @ 3.80 GHz
RAM: Team Elite 4 GB 1600MHz (PC3 12800), single stick
Graphics: ASUS GT 610 1 GB
MoBo: ASUS A78M-A
PSU: EVGA 500w (80 Plus; 100-W1-0500-KR)
OS: Windows 8.1
Monitor: ASUS VN247H-P (1920x1080)

Sounds like you've got a faulty graphics card. Those GT 610s are very low end cards. I had a 620 2 GB and it was awful. I had to take it back to Best Buy 3x before I finally just said the hell with it and bought a 260x for my old rig. With a 1080p monitor, you're probably exceeding what the card has to offer. It may even be overheating.

My bet goes that it's just not able to keep up with 1080p. Try a lower desktop resolution and see if that solves the problem. Set it at 1260x720 and see if that solves the problem. Make sure you do this in the desktop settings as well as the in game settings.

You need to clean out the space between your fan and heatsink on the graphics card. I assume it is too late for this person with the date of the post but this is most likely the problem. Clean out dust remove blockage graphics card will not overheat and cut signal. Everything will continue running after signal loss to continue cooling the gpu this is normal.
