GTX 560 crashes

I've had this graphics card for a little over six months.
When I first got it this problem was pretty infrequent (max twice a month) so I didn't bother too much about it. But now it happens every day and its becoming a nuisance.

Sometimes I get a display driver has stopped working and has recovered (this seems to happen regardless of tasks i.e. games, videos, rendering, and mundane tasks). This is preceded by a prolonged black screen and a burst of fan speed. Sometimes the computer won't show the error but will subsequently restart itself unsuccessfully and require a manual restart.

Specs
Case: K5-715b Gaming case with fan
Additional case fan: Arctic cooling f12 120mm
Motherboard: Gigabyte GA-H61M-DS2
CPU: i5 3570k @3.4Ghz
PSU: Corsair 430w
GPU: GTX 560 1GB
OS: Windows 7

GPU temps (using Expertool):
55 degrees idle with 66% fan duty (seems a little high - not sure why or how to rectify this with limited space for additional fans)

If anyone could help me identify the problem, and whether I need to look into some new hardware would be appreciated. Thanks.
