Are you running some sort of GPU monitoring/overclocking application such as MSI afterburner/GPU guru or the like?

I experienced what you are describing when i had an unstable overclock set by one of these applications, disabling them might help.

I would also recommend checking for the latest drivers available for your card, chipset and even a BIOS update can rid some GPU tasking issues.

MSI after burner can cause some issues. While being the most popular software, if the slides are not at factory default it can mean you are running an "unstable" overclocked value. Try uninstalling it and running without it for a while, see if there is any change :)
