My current pre made pc is this http://h20566.www2.hp.com/portal/site/hpsc/template.PAG...

I'm looking to play modern games at medium at the least. Would I just need to upgrade the graphics card, cpu, and memory? Is that all thats needed?

Sub Question:
If i chose to upgrade my graphics card only, would it be simply a matter of buy a new graphics card + PSU and done?

Unfortunately Intel don't make CPU's for that socket (775) any longer and DDR2 is also no longer made (and as a resutl can be expensive to buy).

Realistically you should look at building a new machine, it's quite possible on a fairly low budget nowadays and would be an order of magnitude faster than the current machine.

It would be best to get new pc, but you could try only with new gpu and psu, and see how it works.
Cpu will bottleneck gpu, but you'll see huge improvement over integrated gpu.

This one will be enough for Medium-High settings

Here's link for Psu, if you don't need modular one, you can save about 15$

Yeah a new Computer is kinda in order :p  but until then I would get a decent psu and GPU(even if it will be bottlenecked by the rest) and then you can upgrade the rest later.
