Windows 8.1 restart and Nvidia gpu failure when gaming

Hello, I've been having trouble when playing Civilization 5 on DX11. My computer often restarts on its own when playing, and my graphics card has kernel mode driver problems and reboots sometimes. It is an EVGA GTX 760 with the latest driver. Also, sometimes when I manually restart my computer, or when it restarts itself when gaming, no data is sent to my monitors at all. My monitors are hooked up to my GPU in the back, using HDMI and VGA ports. Thanks for the consideration! My parts list is linked below.

Do you have access to another PSU you can try?

Could be a lot of things, but off hand the CX line is their entry level and has cheap caps, have seen a number of them that don't provide full power, and the CX 600 actually only puts out 522 on the 12 volt rail....could also try uninstalling and reinstalling the latest GPU drivers, ensure Win is up to date, ensure DRAM is running on correct settings
