Artifacts on Zotac GT 560

PC specs: Intel core i5-2500k, 8GB DDR3, 850w thermaltake psu, Zotac GT 560, my tower is HAF-922 liquid cooled core with a 4 fan setup.

I have (i thought anyways) really good heat dispersion and temps were always in the normal range as I do monitor them with real temp. I've also checked my power supply by swapping it with another pc and everything seems in order. I've had this pc going on 3 years exactly tomorrow. Not sure if it is a coincidence but Thursday morning I updated my nvidia drivers to 344.11 and around 5pm I was watching youtube when all of a sudden my screen looked like a ms-dos game of pong (see pictures).

First thing I did was a clean driver re-install of the previous driver that was working 340.52 and same issue. I next tried a different monitor, different output on the card (vga and hdmi), different settings via nvidia control panel, different pci slot on my MB and all same issue. Last thing I tried was a clean re-install of windows 7 and same issue right on boot without any drivers even being installed. I figure this card is a goner but any help/advice would be appreciated since i'm out of ideas.

Its failing. Count your pennies and start planning a new purchase.
