BF4 freezing/lag please help

Since the early June patch every time I join a server and try to play bf4 when the screen goes from loading to black transition it starts to freeze up and stutter. Then for the first 1-2 minutes the game is unplayable because the frames are so jumpy and freezing. It then settles down but every so often it freezes up again!! Sometimes it has 100% cpu usage, other times high ram, or 100% disk usage or sometimes all 3 of them. Could really do with some help as I've tried lots of things to fix it. Thank you.

AMD FX 6300 3.5GHz
3GB HD7950
1TB Western Digital
8GB DDR3 1800mhz

well, many things could have gone wrong but I suspect software.

It's hard to isolate but if you have the time and a spare hard drive/SSD you could try this.

Turn off your PC, unplug your hard drives/SSD and plug the spare hard drive/SSD. Install your usual OS, install your motherboard drivers, GPU drivers, your usual antivirus and download and install the game. All in all depending on your internet speed this whole thing shouldn't take you longer than 2-3 hours.

After you've done all this start the game and see how it goes. If all works well then you can be 100% sure it's software related and can start trying to figure out one by one what the problem could be. Just a few possible software related things to look into - GPU...
