Graphics card displaying horizontal white lines ; won't boot up

I restarted my computer with my Nvidia GTX660 that hasn't had any problems in the last year and all of a sudden these weird vertical and horizontal lines started to appear on the boot. After the bios screen the screen just turns black and there is no image, I have tried to rollback the driver with no success. I also tried a different monitor and that doesn't seem to be the problem either. I used onboard graphics and that's the only way I can log on to my PC. What can I do to fix this OR can i send it to Nvidia and get it replaced, I don't have the original packaging if that matters.

The problem is most likely a problem with your GTX 660 or is driver related since your onboard graphics work fine.
