GTX Titan Voltage throttling

Recently I was overclocking my Gpu (Asus Gtx Titan) and I'm having an issue with my voltage decreasing as I increase my clock speed. I'm having trouble getting a stable overclock with the voltage so low.

The voltage that GPU Boost 2.0 settled on was 1.162V at 967mhz on the core, the only thing I did was increase my fan to 75% to keep the card at ~66 degrees and prevent the card from throttling at 80 degrees.

I'm going to use +125mhz offset to the core as an example of whats happening. (I'm using EVGA PresicionX)

Voltage - +0mv
Power Target - 106%
GPU Temp Target - 95 degrees (priority)
GPU Clock Offset - +125mhz
Mem Clock Offset - +0mhz
Gpu Temp - 66 degrees (load temp)

Now using GPUZ and PresiconX on screen display (Playing Ryse: Son of Rome) I noticed that my clock speed and voltage bounce around a lot between the following:

1.162V - 1084mhz
1.150V - 1071mhz
1.137V - 1058mhz
1.125V - 1045mhz

The clock speed always matches the voltage shown and its almost always 1.137V at 1058mhz or 1.125V at 1045mhz.

If I continue to increase my clock speed I stay at roughly 1.125V but at a higher clock. For example +150mhz offset.

1.125V - 1071mhz
1.112V - 1058mhz

I get the same overclock but with less voltage and the game crashes after about 1-5 minutes, Understandable...

Would love to get some help with this issue, any suggestions will help a lot. Thanks.

WHAT I HAVE ALREADY TRIED--
Enabling KBoost
Disabling windows UAC
Increasing voltage
Using Asus GPU Tweak and MSI afterburner
Set power management mode to Prefer maximum performance
Link State Power Management set to off

MY BUILD--
i7 3970X
Asus Sabertooth X79 Motherboard
Asus GTX Titan
Corsair HX 1050 80Plus Gold PSU
G.Skill Ripjaw X 16gbs 1600mhz
Corsair H100i
Thermaltake MK-I case (well ventilated, shouldn't be a problem)
