Can someone explain to me what causes my screen to show these artifacts?

I have no idea what these are actually called it would be nice to know what they are as well

    GTX 780 Ti slightly overclocked
    344.11 WHQL drivers
    AMD FX8120 4GHz

below are a few examples of what I am trying to deal with
this also happens in games occasionally, although it is mostly in my browser (Mozilla Nightly 35.0a1 )

a gpu will error out like that for three issue...bad memory on the gpu card...heat issue...bad fan or in game the fan not spinning up to speed to cool the gpu down or lack of power to the gpu. it happen if your using a low end power supply.

is there a way I can verify whether or not my GPUs Vram is okay?
I am using an NZXT Hale 90 1200W psu. I dont know where NZXT psus stand on the quality vs crap scale
