BSODs and other errors with Gigabyte R7 260

I recently purchased the Gigabyte R7 260x (model # GV-R726XOC-1GD) and installed in my system. It would only run a short while before it would BSOD. I tried several things to get it to work including reinstalling the drivers and using a driver cleaner to do a clean install.

Eventually, I came to the conclusion that the card was not playing well with Vista so installed Windows 8.1. This seemed to solve the blue screen issue but now if I run a video or a game or sometimes when a web page is loading, the screen will freeze and either recover and display the message "Display driver has failed and has recovered" or go to a blank screen and my monitor turns off. Sometimes disconnecting and reconnecting the monitor will solve the problem.

I've again tried clean installs of the drivers, unparking the cpu, adjusting the TDRdelay to 8 seconds, and lowering the clock from the factory overclock to the default clock. Since I unparked the cpu, occasionally the error message is "Application blocked from accessing the graphics hardware."
