Format an SD card in RAW format

It's a 32 GB sd card. I recently got it from a friend, and there were issues while I tried to copy files onto it(from a computer).There was a lot of freezing and failing. And after I removed it once, my computer wouldn't detect it again.
So, I tried to format it from my android phone, which apparently failed too, because it says "blank sd card or has unsupported filesystem" now. Tried to format if on my computer, and it doesn't work either. How do I recover my sd card now? I mean, there was no data on it, so I just want to get it working again

Here are the things i've tried:
1. Tried formatting from my computer. Failed.
2. Tried formatting from Disk management, failed.
3. Tried using SD Formatter 4.0. It said the format was completed, but nothing changed.
4. Tried from command prompt (diskpart--clear). Which didn't work either.

Also, here's what my Disk management looks like right now:

Solution
1) Try booting the computer into safe mode with SD card connected and format there.
2) Try formatting the card in other computer/laptop (not in mobile device).

I'd tried formatting it from my laptop, it didn't work. Anyways, another friend of mine tried it from his Windows 7 (I'm on 8.1) and said it was a breeze. Weird.
Every operation on it fails from my pc though [gives an error 0x80070022: The wrong diskette is in the drive]
