100% Disk Usage (tried most solutions already!)

I'm using windows 8.1 on my HP Pavillon and recently it started to get laggy very randomly (I'm no computer expert so I didn't know why this was happening). I have it for a few months now and the lag spikes are becoming far too frequent to even use the device. After booting up the laptop it lasts about 3-4 minutes before completely freezing up and I have to force shutdown using the power button.

I tried:

-Disabling Superfetcher and Windows Search
-Manually setting page file size
-Refreshing the PC (Although this didn't actually work, it kept saying "error occurred and no changes were made".
-Uninstalling Google Chrome
-Setting EnableSuperFetcher DWORD to 0 and Prefetcher to 0

Anything else I can try? I need my PC urgently for University!!!

Have you run disk diagnostics to see if there really is something wrong with the HDD?
