Recent windows 8.1 build running super slow

Hey guys, I'm hoping one of you can help me out. I recently did a build and put windows 8.1 on it. It ran beautifully for the first 5 months but recently it has been taking super long to boot. I would say about 5-10 mins long before I can actually open programs. After that time then it will run great an fast as usual but then randomly it will hit a wall and I have to wait another 5-10 mins for it to gather itself.

What I have done so far:

Drivers are all up-to-date
Defrag HD weekly
Malware removed and Virus free
Disk clean up ran
Cache Cleaned
Disabled most start up programs
Recycle bin emptied.

All the very basic things done. My RAM is all being recognized and nothing seems weird on the hardware side but I'm by no means an expert on hardware issues. Any help would be wonderful!

What are the specs on the machine? Sounds like a hard drive issue to me. Grab CrystalDisk from here and see what it says. Be careful when installing this, though, as the installer has OpenCandy. Make sure you don't install anything but CrystalDisk.

Let me know what your specs are and what CrystalDisk says.

WD Blue 1 Tb
Antec 450 watt Power Supply
Gigabyte F2A88X-DH3 Motherboard

I'm not familiar with Crystaldisk or any SMART software so I'm not sure what I'm supposed to be concerned about.

In the reading it says I have a "caution" health status in both C5 Current pending sector count and C6 Uncorrectable sector count. If you know what these mean that would be awesome.
