GTX 560 Ti problems

First of all hello.
So ill explain my problem. My card is about 3 years old, its palit gtx 560ti and its core clock is increased to 900 MHz by default. When I ran it first week I had some driver crashing problems with it on some games. They freeze then black screen and ballon pops informing about driver crash and recovering from it.
So to avoid that I lowered my core clock to 852 MHz. From that point all good until recent crashes. About 3 weeks ago I was playing League of Legends and my screen goes bad, all kinds of bad spots, pixels broken and so on. At first my idea - VGA dead because even if i restart my pc they stay when bios is loading. Managed to fix it by reinstalling my win7 so blamed the crashes on drivers. For two weeks it ran ok, no glitches or crashes.. And today while playng League of legends similar screen glitches occured. After those blue screen of death and restart. Glitches did not occur during win startup or bios as before they did. But now it did show the driver has stopped and recovered from error. So I tested it with MSI Afterburned what happens. Just before crashing gpu usage jumps to 70-99% it crashes and recovers. Those jumps are when im doing nothing, just watching afterburner stats it goes from 0% usage to 70-99 and crashes. I did reinstall all again, did not help, lowering MHz does not help. What helps is uninstalling nvidia drivers and crashing stops. Installing older nvidia drivers does not help. Also changed thermal compound for VGA and CPU today.

Pretty messy stuff here, sorry my english isn't the best. Looking for some answers here, is VGA dead or the problem is software based?

wait wait wait!!! you overclock it hahahhahahaha thats the problem bro never do that it will burn you graphics if it has it dosent mean you should do it listen the best way to fix it is format hard drive and install new windows then restore everything in bios to its orginal settings and dont touch bios man if you touch bios will be dead

It was overclocked by default...

It's factory overclocked though. nVidia overclocked it. It should be stable.
