Power supply or Graphics card issue?

Ok so I have been having this problem recently, I thought nothing of it the first few times it happened but now it's become more frequent:

During a game I have a problem where it appears to freeze a couple of seconds, the graphics card then powers down (I can hear the fans power down) the screens go black and then after anywhere between 5 and 30 seconds everything comes back to me, I've experienced this in Counter Strike and Guild Wars 2, my system plays both games perfectly fine and I'll post the specs at the end.

I cannot decide if it's the PSU or the Graphics card (or maybe another problem). Before It was fine, no problems what so ever but my PC is now 2 (maybe almost 3) years old and the power supple is as old as 7 years (from a previous custom build).

Specs: (off the top of my head)
i5 3570 K
8gb RAM
Zotac GTX 660 ti AMP!
Corsair 620hx PSU

(can't remember the motherboard off the top of my head but I believe it's an ASUS MAXISMUS V Gene 3 ... I think)

Anyway, any help would be appreciated.

It'll still consume pretty much the same power. Maybe 5-10W more but nothing significant.

I was planning on doing an upgrade to something a bit bigger soon anyways around a 750 just to play is safe (very safe). Just hoping it's not the GPU, it doesn't struggle to play any games and that's been it's only issue thus far.

But I thought I should point out that I am using two screens - I don't know if this'll have much of an impact.
