No idea what is wrong.

So I built my custom desktop a while ago, and boy have I been having problems. I've made a couple of posts here in the past, but recent events have lead me to making another post.

Note that there are two HDMI outputs for my computer: the GPU and the Motherboard. Normally, I use the GPU's HDMI. I have Windows 8.1 installed on the SSD.
But sometimes this happens.

I'll be playing a game (recently Hearthstone, last night Dragon Age Origins, or just 5 minutes ago I was trying to type this and it happened), and all of the sudden my screen goes into "HDMI Power Saving Mode." Which means 1 of 3 things has happened.

1) My system power cycles and boots up as if I had pressed the reset button (ASRock logo appears on screen, then Windows 8 load screen, totally normal process).

2) My system power cycles, but the screen stays in HDMI power saving mode. I then unplug the HDMI cord from the GPU and put it in the Motherboard, and I am greeted by the Windows 8 login Screen.

3) My system turns off, and neither output displays anything (the lights in my case are still on, and my fans). When this happens, my XBox controller which I leave plugged into a USB port blinks a ring around the middle button (normally it solidifies and lights up the "player 1" light).

Whenever my system is working with the motherboard outputting instead of the GPU I don't stress it too much. It never seems to crash when I am doing this. (Also side question, if I am outputting via the motherboard, is the GPU still active in Graphics processing? Or just the Intel HD Graphics card?)

I have had this problem for quite some time. I have already gotten the PSU and the Motherboard replaced (I had Microcenter replace the Motherboard for me), so that leaves me suspecting I have a bad graphics card. Is there something else that could be wrong?

Also I recently had Microcenter look at my computer and they agreed with me that my motherboard was the issue so I had them take care of that, but this is all still happening so do you think there is a way for me to get some money back here? I had to pay for diagnostics, installation and the new motherboard.

looks OK. I don't know why the z97 mobo though. And I'd've preferred a better PSU

What monitor?

Try it without the XBox controller.

You should have " One Dual Link DVI-I, One Dual Link DVI-D, One HDMI, One DisplayPortStandard Display Connectors"

Try it via DVI-I to VGA

I don't think the XBox controller has anything to do with my computer crashing like this...I just mentioned the controller because I thought the blinking pattern was distinct enough to maybe signify a specific problem. But if it makes you feel better I will unplug it (I only use it to play games Wii/Gamecube games on Dolphin).

And what PSU would you recommend? I don't even need 400W for my build so I would think that my select PSU should work just fine. But if you really think that could be the issue I am more than willing to buy a better PSU if it will solve this problem.

This is my monitor if you think that matters...I have had my laptop plugged into this guy for a year before I built my desktop and it's worked fine.

The next time it does that I guess I can try the DVI output, but I really don't see how that would help the power cycling...

Just this morning I was playing Hearthstone and I had Chrome open. Computer "crashed" (rebooted, whatever) and monitor entered HDMI power saving mode. So I plugged into one of the DVI ports and used my converter dongle to connect to the VGA input on my monitor (I don't have a DVI cable). And when I switched the input to VGA on my monitor it just entered D-Sub power saving mode. I plugged in the HDMI cord to the motherboard, switched to HDMI input on the monitor, and Boom there was the windows 8 login screen.
