Freezes, Crashes, Hangs, BSOD, ATAPI 11 error

First, my pc is 6-7 years old.
I have serious problem with my PC for about 5 months. It all started when my old GPU(Nvidia GeForce 8400) died 8-9 months ago. After that I bought a Nvidia G210, but after a few months it started to hang while watching videos or playing games. Then I bought a Nvidia GT610 and I still have it. The first days it was all good, but after a few days it started to hang too. At first the hangs were coming after 5-6 hours playing, but as the days were going, the hangs were more and more recently. Now my pc is slow and when I open a game (like dota2 or counterstrike) it freezes. It is freezed for about 2-3 mins and then in the most cases it shows a BSOD, but sometimes it unfreezes and again freezes after 1-2 minutes. I saw on Hardware Monitor that the 12V shows about 9.5Volts so I thought that the problem is the PSU. Then I changed it with another one (the other one is old about 4 years but it has not been used) but the HW monitor was still showing 9.5V where should be 12V. I wanted to make sure the PSU isnt the problem so I tested it with instrument and it showed good for 12V. I guess the motherboard controllers are showing wrong value. I tested the RAM sticks with memtest and they showed well. My case is perfectly clean and cool so it is not a cooling problem. I bought a new hard disk and tried with another sata cable but money wasted for nothing. So it is not hard disk issue, not cooling issue, not memory issue, and not psu issue I guess Also I formated and reinstalled windows 7 and 8.1 a few times.
Last days I checked windows logs and I saw that there are a lot of Atapi 11 errors " The driver detected a controller error on \Device\Ide\IdePort2. ". I was searching the internet and many people have the same problem with atapi error 11 and freezes, but I didnt find a solution.
Also 1 more thing- the pc doesnt detect the pri-master and my only hard disk is third ide master, also sometimes when I boot the PC it says " some error with hard disk, press ctrl+alt+del to restart" and it sometimes the PC stucks at starting windows.

UPDATE: Now I reminded that 2-3 times when I shut down the PC, the coolers were still working and I had to pull ou the power cable to turn it off.
