Anyone know this game? (OLD Car racing/shooting)

As a kid I remember playing games like road rash, the test drives, Apache, and worms 2... but there was one game that I can NEVER remember the name of. I've searched for this friggen thing for years, and no one seems to be able to find it.

Here is the information I have about it.

It was a third person (with optional FPS) car game. There were different missions or custom game settings that would enable you to simply race or not use guns at all, but the game revolved around adding weapons to your vehicles. They were not fancy futuristic vehicles, all the vehicle models were actual IRL cars. Some of the weapon options were machine guns, flame throwers, and rocket launchers.

Campaign info: The most valuable info I have about the story line, is that the lead character is trying to hunt down whoever killed his girlfriend/sister. Near the beginning of the game, there is a cut scene in where the girlfriend/sister in question is investigating a house (on foot). A pair of headlights (and engine sound) appears behind the house and the car runs/guns her down.
The lead character then reminds whoever he was talking to... (After the flashback) to "never, get out of your car".

Another mission; You go to ambush a rival "Gang" and when you get there they are waiting for you. The main guy you're trying to kill is in a white station wagon/suv and his main weapon is a flame thrower. The mission is at night, so it was quite a vivid mission. After this mission is when you gain access to the flamethrower for your own personal vehicle.

Another mission; you and your gang are behind a race track/horse track and you jokingly start racing trying to beat each other. After the race you get ambushed by a group of enemy's.

Another mission; You're in a desert with one partner, and there are "japs" in hatch backs with a "rising sun" color scheme draped across their cars.

When you would shoot the machine guns (the most common used gun), it would make these ugly yellow lines "down range" which were supposed to be the bullets.

When you were driving in FPS mode, when you pressed the "glance" key to either the left or right you would produce a hand gun that you could shoot from the window.

If you made a "custom game" and went up against other players or bots, you could fully customize any of the vehicles to include whatever you wanted (if the attachment was compatible). During this customization phase, the menu was on grid paper with hand written text with "blue print" type pictures of the vehicle you chose. You could change the vehicles engine size (v8, I6, v6, v10, even v12, etc). Armor plating (didn't show up in-game but the values were present) color, paint scheme/design, gun placement.

The game disc was either all orange or all blue. There was a number repeatedly printed on the disc, like... in sequence to fill the disc with the number repeatedly. The number was something along the lines of either 70s, 60s, 80s, 90s, 77, 66, 77s, etc.

The "install" or "repair" disc was either all orange or all blue. With the same number repeating across the disc.
