hard drive does not show after reboot

asus g75vw 32 gb ram 256 ssd running windows tech preview build 9879 second hard drive the original 750 gb I see in disk management as unallocated. After formatting it shows up in windows explorer with its full volume. When I reboot it disappears. When I go back to disk manager in says it needs to be initialized when I go to do that choose gpt and it says partition to small. If I choose MBR I get an I/O error.

Right now it is showing as Not initialized. I can't initialize it. How can I repair this disk. I have tested the disk by installing an operating system and it works fine. Then I clean the disk and same thing. First it shows as unallocated then I format it but when I reboot it returns to Not Initialized.
