Graphic Card upgrade suggestions?

Alright, to explain as simple as possible I'm done with console gaming and want to go the PC gaming route. The below is my setup:

Asus M5A78L-M LX
Visiontek 4GB Ram x2
AMD FX-6300 3.50 GHz
Crucial MX100 256GB SSD
XFX Core Edition Radeon R7-250X 2GB
500W (PSU)

I want to upgrade so I can play games like Battlefield 4 and GTA V on Max Settings.

In all honesty, I think you should go for the R9-280. In another machine, I have a Phenom II X4 965BE @ 4.0 GHz using a Sapphire Dual-X R9-280. The Phenom is right at its max with the 280. Any faster card and it would be bottlenecked by the Phenom II. Your Fx-6300 is in the same class for gaming as my PhII X4.

And, the R9-280 can always be OC'd to 280X speed.

I just checked and it's actually Cooler Master Elite Power - 460W Power Supply (RS460-PSARI3-US)

What's your budget? I think your upgrade route should go GPU > PSU> Motherboard > CPU cooler.

GPU is what will effect your game performance the most. Maybe a r9 280 for $200? In which case you'll need a psu upgrade to handle the extra power. In the future maybe a new mobo and CPU cooler for overclocking.

Well I'm not into overclocking and most likely wont do that but I won't rule it out. My budget I would say is $400. I was considering just grabbing the R9 290 and a new PSU?

That is a rather poor PSU. I would not keep that if you plan on using a faster card. Its 2 little 18A +12V rails are more like something you'd see on a 300W PSU. If you want to use a R9-290, you will need a 600W or better PSU. One of these should be good for you. You may want to consider dropping back to the R9-280/280X to avoid bottleneck unless you plan on OC'ing the FX-6300.
