GPU black screen, max fan speed

Black screen+max gpu fan is common, but I suspect it's a death of my graphics card.

i3 2120, ati 6850hd

Played a game(far cry 4, played for days before, it was fine), and then it happened, maybe 5mins after turning it on, turned off pc. Tried to run it- no video, but then switched to vga and it was fine, but after 30mins it happened again, but this time pc was idle, just chrome browsing. Then no video, no matter whether hdmi or vga, waited a little and after 1min gpu went to max speed anyways. Then, plugged my monitor to intel hd 2000, but didn't take a card out. It was fine, but after 10mins my graphics card fan went crazy again(which is weird, as it was not even working?...just spinning fan). Took a card out, and it's fine, but I'm curious- is that a dead graphics card? Last drivers were installed 6months ago, I don't think it's software issue.

Most likely is that you overpushed your graphics card.At somepoint ingame the graphics card couldnt handle the pressure and died..im not sure tho..try ask a moderaitor or smthing.

It was about 75C when it happened the first time, but it had been fine for days with up to 79C gpu. It's obviously not temperatures fault at all, it just could have caused a failure, that's it.
