Can't open micro sd because it needs to format the disk

i use a 16gb micro sdhc class 4 for my r4 card. but for some reason when i wanted to look at the files on my computer says "windows needs to format disk" but when i try to format it, it says "windows cannot format the disk" Any Help?
also if i an able to can i retrieve any of the memory on it?

Go to safe mode and plug in the card. See whether you can view the files. If not, right click on the drive and select properties. Then go to Tools tab and click "Check Now". Select both the options and start the checking. It'll take some time.
