Please help with my chkdsk read

My computer is having some problems for a while. It is *very* slow sometimes, and get stuck so I need to hard reboot it. It happans a lot, and especially after hybernation. So, I made a disk check. What can I do? this is my disk check log:

I recently formatted the computer.
I have 562GB of free space.

Thank you very much for you diagnosys.

I also received a windows messege alert the morning after the dskchk, saying "windows detected a hard disk problem. Back up your files to prevent info loss, and contact the comp manufacturer to determine if you need to repair or replace your hard drive"

I hace everything backed up. I think it's inevitable to replace the disk. I'm using HP Pavilion g series. How can I know what hard drive I need to purchase?

You can either contact HP support with your model number for that information, or take a look at the size of the drive.

My educated guess for the size of your HDD that you need to buy is any 2.5" drive.

So look for a 2.5" form factor hard drive, SATA, and I can almost guarantee that it'll work in your system.
