Need help diagnosing M.2 SDD / Win8 inconsistent boot problem

I built a new desktop PC about 2 months ago and have been having inconsistent boot problems.

Short Version

2 out of 5 times my desktop locks up on boot, it reaches the Win 8.1 blue boot-up logo then locks up. But when I get a clean boot the PC appears stable (passes stress tests etc.).

Long Version

Main components:

MB: ASUS RAMPAGE V EXTREME, ATX x99 (latest BIOS installed)
CPU: Intel Core i7-5930K (not overclocked) with Corsair H100i cooler
Memory: G.SKILL Ripjaws 4 series 16GB DDR4 3000 (PC4 24000) F4-3000C15Q-16GRR
OS HD: Plextor M6e Series 512GB M.2 PCIe SSD (PX-G512M6e) (1.05 latest firm ware flashed)
Power: Corsair RM Series 1000 Watt ATX/EPS 80PLUS - CP-9020062-NA RM1000
GPU: ASUS Graphics Cards GTX780TI-DC2OC-3GD5 (x2) (Lastest Drivers and BIOS)

Symptoms:

5/2 times on startup boot it locks up win 8.1 (blue windows icon) screen and the little rotating icon does not even start rotating. Once I get a clean boot, the PC is stable (I’ve ran CPU {Prime95}, Memory and HD stress tests and can game for 8 hours stretches with NO issues whatsoever).

MB info during failure:

(varies) Post Message, I have seen various (not consistent) 00/61/b7 Is there some trick to know if the message is a "b" or "6" ... wonder if I am reading it wrong.

(sometimes) The MB diagnostics LEDs: Lane 4 flashes – this the PCIe 3.0 x8_4 slot which is shared with the M.2 Drive. This has me very worried that its the MB or the M.2 SSD ... but its not consistent, it can lock up (same place as always) and this light can be off too.

(sometimes) the BIOS after fail + manual rest said failed overclocking (even though I have disabled ALL overclocking)

(sometimes) After a failed boot + rest, I get Win8 repair screen, (sometimes) after click restart it would boot, sometimes it locks up again Win 8 logo. (sometimes) the repair screen would show but just like the normal boot logo would freeze up.

(so-far only 3 time) got “blue screen of death”, x2 playing Dragon Age (exact same in game location, during HD activity). Once during backing up M.2 SSD OS image to secondary HD. After all 3 the BIOS msg was “ASUS power surge detected”. The x2 reboots same location in Dragon Age location makes me want to dismiss power surge msg.

Once booted into Win8, the machine is VERY stable. I have run memory tests and CPU stress testes. No crashing, stable. No heat issues that I can tell, though I can measure M.2 SSD temps. But this issue is right at boot, even before heat build up.

What I think the issue is:

In order of suspect, either: (1) M.2 SSD, (2) MB, (3) windows 8 secure boot related (UEFI) or (4) power-supply.

This issue is driving me crazy ... I LOVE the build and the M.2 SDD is really really nice.

The fact that I get to the Win 8 logo really makes me think its not the M.2 SDD ... that and once I get into Win 8 I can game/work for hours no issues. Its just at boot time :(

What I have tried so far:
1. I have re-seated the M.2 SDD, power cables, everything ... just making sure nothing was loose.
2. I updated all drivers (windows 8), BIOS (asus) and firmware on MB, M.2 SSD (and on all my other hardware components).
3. I have disabled ALL other booting options in BIOS, I have select windows boot manager.
4. I have enabled/disabled CSM (Compatibility Support Mode)
5.I have tried disabling Windows UEFI in BIOS

I am at the point where I want to install Win 7 and see it I have better luck.

Any advice would be most welcome, I am at wits end.

I resolved my issue, weird as it was X99 platform apparently does not always play well with older USB devices. I had a CH Products Peddles also connected, removing it totally resolved the random failed POST and seemingly random Q code errors. My system is now 100% stable at boot and in OS. There is an option in the BIOS that you can disable USB 3.0 - or do what I do, I only plug the device in after boot when I want to use it.
