Mini-ITX VESA mount PC case with external PSU and PCI-e slot

I am attempting to build a PC that can screw into the back of my 27" monitor and house the following components:
Intel Core i5-4690k / MSI GTX 750Ti (low profile) / Mini-ITX mobo / 2.5" SSD
I have looked high and low for a VESA mountable PC case that can house these parts but also
1) has a 350w external power supply
2) no optical drive space
3) room for a low profile discreet GPU
Any suggestions would be greatly appreciated.
Oh and it needs to be able to support itself with just the VESA mount, no additional bracing required.

If you mount a system with a motheboard and video card in it, it will very likely tip over the monitor or make it very wobbly without adding the same amount of weight to the front of the monitor.

Plus there is no case that will fit a 750Ti card that has VESA mounts on it.

What you can do is get a small case and then mount a VESA plate on it.
