windws is unable to format sd card

when i put my san disk 4 gb sd card into my pc you can only use it by format. although it is 4 gb but it show 32 mb raw file and . after clicking the format option it says windows was unable to format this drive and make sure the drive is not read only.

I had one like that once (a micro sd), gave it to my wife and it started doing that when it went bad. You could try doing it from the command prompt, or run chek disk on it and see if it fixes it.
