Game is unplayable due to stutter/FPS drops. Any way to fix?

I'm talking about Watch Dogs.
My PC:
Sapphire HD 7950 3GB Boost (1200MHz/1320Mhz OC).
8GB of RAM.
i5 2500.
SSD 120GB.
1680X1050 RES.

The game is literally unplayable because the framerate drops and stutters so frequently. I have a 7950 (and yes I have updated to the latest drivers) running the game on High; this definitely isnt a hardware issue its a game issue.
Any possible fix for this?

Yeah I havnt played it on PC, but I have heard that it is one of the worst for optimisation.

What is your anti aliasing on? if it is on something like 4xMSAA or 8xMSAA try turning that down, that will significantly boost performance.
 
The game should automatically update through either Uplay or Steam depending on where you got it. If not, try to manually trigger an update for it. Even then, you may not get great performance, apparently the game just runs terribly on AMD cards, and that has been an issue for pretty much all the 2014 Ubisoft games that use Nvidia Gameworks. Bottom line is, don't even bother with Ubisoft games on PC anymore if you don't have an Nvidia card.

As supernova said above, the quality of Ubisoft games recently has been garbage look at ACU for example, and a note about the Nvidia card thing. It also has to be a pretty good card. A friend of mine has a 650 ti and only on Assassins creed and another Ubisoft game that I forgot it runs slow and crashed every 10 mins or so.
