Blue Screen of Death, Random Restarts, and Freezing. Need help.

Over the passed few months my Computer has been running into BSoD problems left and right when gaming, and I've tried just about everything I could think of regarding the BSoD errors I've received. I've reformatted my PC multiple times, I've run MemTest to check my ram (10 Passes and 0 errors.) I've ran a diagnostic on my HDD, Updated EVERYTHING I can think of to the latest drivers (Including Chipset drivers). I do have WhoCrashed downloaded, as well as BlueScreenView. Most BSoD tend to have "Kernel" in the error and everytime I've googled the causes of the errors seem to stem from either Driver related issues or RAM issues. I've tried and tried to solve it, using every possible "Fix" I could find and to no avail it seems to be getting worse and worse as time goes on. There doesn't seem to be a Rhyme or Reason to making it Blue Screen, sometimes it happens often and sometimes not at all. So I'm reaching out for some outside help, and hoping to learn how to deal with BsoD's in the future.

My PC specs are:
OS: Windows 8.1 64 bit
CPU: AMD Phenom II X4 965
Motherboard: Gigabyte GA-890FXA-UD7
GPU: Nvidia GeForce GTX760
RAM: 12 GB DDR3

As of recently it seems my PC just randomly restarts or freezes more than BsoD but the last 4 dump files are (some of these are a result of running Verifier.exe):
DRIVER_VERIFIER_DETECTED_VIOLATION "Caused by driver Mwac.sys." - BlueScreenView (Malwarebytes related?)
DRIVER_VERIFIER_DETECTED_VIOLATION "Caused by driver ACPI.sys" - BlueScreenView

DRIVER_VERIFIER_DETECTED_VIOLATION "Caused by driver BTOWSFF.sys"

Some of the actual errors I've received on the bluescreen itself:
KERNEL_SECURITY_CHECK_FAILURE
APC INDEX MISMATCH
kmode_exception_not_handled (alsso kmode_exception_not_handled (amdppm.sys) )
As well as others that I cannot remember, or the bluescreen flashed too fast to read.

Not sure if relevent but the Game(s) in question are Final Fantasy XIV: A Realm Reborn and Sims 4 (all blue screens happen whileI'm playing either of these)

If there is any other information I can supply that would be of further help, please don't hesitate to ask and I'll try and figure out as much as possible. Any and all help is greatly appreciated, I'm at wits end with my computer and am on the verge of building a new one just to rid of BSoD errors (hopefully)
