What happens if you connect pins 1 and 4 (ground and power)?

Does it have some type of protection against this? What about phone adapters and such?
Will it blow a fuse, fry it, start a fire, etc.?

Thanks,

It's generally not a problem unless the product is broken.

Connecting ground directly to power is never a good idea and what happens in that situation depends on the device being used but likely you'd fry some component inside.

My dad's Playbook died when connecting it to the PC via USB to transfer files. I investigated and found out some of the flat wires had come unglued and shorted together. No smoke, no fire, no noise, just a dead device.

Fuses are useful but they aren't used everywhere.
