I recently acquired this Twin Frozr III card in a box of electronic junk. I tested it, and it worked enough to boot up the computer, but as soon as it got to an OS. Doesn't matter if it's windows or linux, the fans would start spinning fast and the screen would loose signal. Now it's obviously an overheating problem, I'm thinking with the GRAM, because that's where the card is getting hot. I've replaced the thermal grease and thermal pads, but still getting the same result. When I got the new thermal pads I kinda guessed on them because didn't know the exact size that works. It seems like they fit, but the card doesn't work. Is the card faulty or did I get the wrong thermal pads? I believe I bought the 1cm pad.

Sounds like a faulty card to me. If the change in thermal interface with the good cooler as it is it apparently making it overheat, then it might be a thermometer problem. The card thinks that it's running too hot once the OS kicks into gear and spins massively until it turns itself off to avoid damage.

Unfortunately I don't know any fix you can do for this.

I'd say it more of a heat transfer problem. The card tends to get hot around where the RAM on the circuit board. All I can think of is that the thermal pads aren't thick enough.

It takes about 3-5 minutes for this card to overheat, screen turns black and fans spin at their max, after which I take it out of the case and the GPU isn't hot, only the GRAM toward the back of the board. Should I try a 2cm pad? Because it looks like the 1cm works, but I'm not getting the results.
