Two out of three monitors working.

I have three ASUS VE276 monitors connected to my PC with the following specs:
Windows 7 Pro (64 bit)
Intel Core i7 - 3930 @ 3.2GHz,
16 GB RAM,
2x Radeon HD 6990 (in Crossfire),
Intel 250GB SSD HD
2x WD 1TB HD.
I can only get two of three monitors to work in eyefinity, with one monitor connected to the DVI output, and two connected to the mini DVI each through Saphire active adapters. Eyefinity recognizes three monitors are connected, and allows me to select 3x1 config.
All drivers are up to date.
Trouble shooting has demonstrated that all three monitors work, all three cables work, both active adapters work, any variation of mini DVI output has the same result.
Please help.

The output ports of the HD6990 Graphics card are:
1x DVI
4x Mini DVI

Do you mean: get a DVI to HDMI cable and plug it into one of the mini DVI active adaptors? So the intention is to use a HDMI port on the monitor, but using a Mini DVI port from the card?
Please advise

And your card's: The output ports of the HD6990 Graphics card are:
1x DVI
4x Mini DVI

That's why I've asked the specific model of your card.

Ok, I still don't understand why its not working with all three monitors. I had it working before, but it simply stopped, so now I'm trying to work out what is preventing operation with three monitors.

The specs say:
MD Eyefinity multidisplay technology1
Native support for up to 5 simultaneous displays
Up to 6 displays supported with DisplayPort 1.2 Multi-Stream Transport
Independent resolutions, refresh rates, color controls and video overlays
Display grouping
Combine multiple displays to behave like a single large display

Naturally, any ideas are greatly appreciated.

I swapped the graphics cards and all the monitors, and the cables and the adapters, and still have the same problem. The monitors are all exactly the same type, the graphics cards are exactly the same cards (and manufacturer), the cables are the same, the only thing different is that two of the three cables have mini DVI active adapters. The DVI port works, and one out of two of the mini DVI ports. It makes no difference which of the four mini DVI ports are used, the result is the same.
Quite frustrating.

This issue seems to be a common one, and I'd like to share my solution, which I have to thank alexoiu for assisting with.

I found that one of my active adapters had malfunctioned and was indeed faulty. After narrowing it down, I replaced it with good results: three monitors back online.

This goes to prove that eyefinity with three monitors is very particular. I would suggest the active adapters should be the same as supplied with the graphics card, keep graphics cards the same, and ideally the same kind of monitors.

I hope this assists others with similar problems.
