Ok so, I've bought a brand new Asus maximus iv extreme rev 3.0 motherboard and i send the motherboard and my pc to remove the old motherboard and install the new one! However when it came back the graphics card was installed on the second pci-e x16 slot instead of the first! I did a complete format of both my SSD and HDD and i installed the graphics card from the second slot to the first! The pc booted fine but when it started to load windows i got the NO SIGNAL on my screen.. After waiting about 5-10 seconds I got to the screen where i write my pass for windows.. After that it went black screen again and then it said display driver stop working and has recovered.. And then randomly after about an hour it gets NO SIGNAL again.. I then installed it on the second pci-e x16 slot, and it is fine... Is the first slot malfunctioning? Please tell me it isn't...

It Must Be 1'st PCIEx16 Slot Dead But Try Resetting BIOS And Also Check Whether Both Slots Are Enabled In BIOS Settings If Not Then Enable Them

Could it be dead by static electricity? Or came in dead from the manufacturing ?

Hmm.. But if it is dead shouldn't just be, you know.... dead? The thing is it does work but sometimes the driver just stops!
If it was dead it wouldn't work at all

If it is dead, can it be repaired? If yes, how much does it cost?

I don't think you can repair a pcie slot. If you tried, it'd cost more than the motherboard.

If cleaning the first slot doesn't work then try your card in the second pcie slot. If that works OK then leave it there. If not then replace the motherboard - RMA it if its still in warranty.

Well the second works ok, but if i want to SLI i'll have to put the card on the fourth slot and they will get sandwiched.. My thought of placing one in the first slot and the other at the 4th or if it doesnt work at the 3rd atleast :(

http://www.asus.com/au/Motherboards/MAXIMUS_IV_EXTREME/specifications/ says -

4 x PCIe 2.0 x16 (x16 or dual x8 or x8, x16, x16)

So you can run 3 slots out of the 4. It's not real clear on which is which. See the motherboard manual for clarification.
