Computer shuts down, but case fan still runs and LED stays on

I've had a weird problem recently.
I moved my custom built PC (built in the middle of last year ish) and replaced the power cable then after a couple of weeks had the issue where the following happens:

In the middle of playing Dark Souls II, suddenly monitors go dark (but stay powered on). All peripherals have power (even the keyboard is lighting up), but my sound has stopped and as far as I can tell nothing is happening on the computer.
However, the light is still on (blue light at front of case), and my case fan is still running, though none of my component fans are.
At this point my only solution is to power cycle. Have since replaced the power cable again, and the issue persisted (only while playing Dark Souls II, have not tested with other games).

I don't feel any noticeable heat coming out of the case (except the back of the power supply, which MAY have been a bit hotter than usual, I am not sure). Speedfan when idle shows temperatures in the 40-50 C range. Computer has run flawlessly in the 9 months I've had it until now.
Is the power supply at fault, or is there something else that might cause it?

My specs are as follows:

Aerocool VP650 650W Power Supply

AMD FX-6300 6 Core Black Edition Processor

Toshiba DT01ACA100 1TB 7200 RPM HDD

ASRock 980DE3/U3S3 Motherboard

Sapphire Radeon R7 265 Dual-X 2GB Graphics Card

Corsair 8GB (2x4GB) 1600MHz Value RAM DDR3

Any answers will be much appreciated.

EDIT: Computer boots with no problems every time.
EDIT 2: After having it happen again, I opened up the case, power supply did indeed seem quite hot. Not hot enough to burn my hand when I touched its housing, but hot.

Solution
Those are signs of a failing PSU, but cant be 100% sure. It could also be MB, GPU, CPU in that order...
Get OCCT, and run CPU, GPU and PSU tests. See where it fails.

I had that same problem with my R7 265. First, are the fans on the graphics card running when the screen goes black? Have you tried clearing the CMOS (fixed the problem for me)? Have you tried unplugging the display cable and plugging it back in?
