It's a Club 3D 6950 1GB which isn't the best obviously but
say everything runs smoothly for 2 years and it's in a good condition.
Will it work properly for another 2 years? (Good power , Good Cooling and changing the Thermal Paste)

Actually that is a 5 Year Old Card http://www.pcmag.com/article2/0,2817,2374337,00.asp
Hardware warranties are for only 3 years at most, so after 3 years your taking a chance.
And the 6Series isn't even shown on the AMD webpage http://www.amd.com/en-us/products/graphics
Given that the demand of games is much higher than the card can perform, as well as these other factors, it is way over due to be replaced. especially as it is a old PCI Express 2.1 card, and current cards are 3.0 standard.

Will it keep working? It can, but the manufacturer told you in the Warranty, they won't guarantee it does past 3 years.

You know PCI-E 3 does not do much for anything but the absolute highest end cards(dual gpu cards maybe).

To add to this the OP would need a new system(assuming it is older) to take advantage of the 1-2 percent(on average) even on a GTX 980 card.

Lower resolutions have the "largest" difference.
