GTX 970 issue "only 2 displays can be selected" when attempting 3 monitors + accesory display

Hi all! Didn't know if to post this under Displays or Graphics cards.

I'm having a headache-inducing issue with my rig. I have a GTX 970 and 4x Dell P2214H 22" monitors, using 2x DVI-D, 1x DP & 1x HDMI. Every single time I try to configure Surround, it gives me the "only 2 displays can be selected" issue. Even if I unplug the HDMI one and try with 3, same issue, cannot select more than 2.

All displays are the EXACT same model, 1080p 60hz, with the official Dell drivers installed, stock 1080p resolution, and have tried with both the latest 347.88 and 345.20 drivers, even deleting them any trace of Nvidia software via a driver cleaner, and starting again. Same issue persists. The only difference to the screens is 2 of them have no logo and show up as Dell P2214H, and the 2 with a logo show up as the same but in caps, DELL P2214H.

Any idea how this can be fixed? My rig is an i7 4790, 16GB DDR3, and Windows 7 x64. I have tried deleting Nvidia drivers and the 4 latest driver versions, along with formatting my PC and starting over. Still no luck.
