Go back to the 'customize notifications items' window.
Click the blue "Turn system icons on or off".
For "Volume", select 'On' in the drop-down box.

Thanks for your response, but that's not it. The Speaker icon shows up. It is not clickable. You cannot right click to pull up the sub menu for Playback Devices or Recording Devices, etc.. You cannot left click to bring up the volume slider and mute button.
I found a registry fix that corrects the issue for the current user logged in. But I have multiple users, and 300 machines. I can push the batch file to correct for one user, not when anyone else logs on to the machine, the problem is still there for them.
Even when I apply the registry fix to Classes Root, Local Machine, Current User, etc.. in all sections of the registry, it only applies the fix to the current user logged on, even if logged on as local administrator.
