Radeon HD 6950 (GIGABYTE) Graphic Card crashes !!!!!!

Hi, for long time my screen is crashing, and it freezes as seen on the link. There occurs many little squares on the screen. I tried each output of the graphic card and the results are same. Whether DVI or HDMI, each one crashes. Actually even when i firstly turn the PC on, at the beginning, before the Operating System boots, it is crashing anyway. I couldnt find any correlation about that, it can be crashed randomly anytime. A few seconds later after this little squares, it turns into dark black screen totally. Then I reset the PC, sometimes it works well but sometimes i need to reset it 5,6 times in order for PC to run as usual. My motherboard is GA-870A-UD3. I even tried to change the slot of the graphich card on motherboard, but the problem existed anyway. My driver is also up to date. I am giving 2 links for 2 different figures, one of them is the crashed screen and the other one is from conflicts/sharing menu. I hope these information would be enough for a good observation. By the ways without any changing in hardware, the PC was running pretty well till that day. But suddenly something happened with my graphics card. When i changed this graphics card with my previous old card, the problem disappers. What can cause this problem, is there any solution to that? If u help me with this issue, i would really appreciate it.

try to go back to the 13.12 driver ?? I out of the blue had something like that with my 7850 card on the 14.12 was seeming fine then one day screen went black with the no signal balloon -- so after fooling around I got it in to safe mode and I figured the card wen t bad I slaped in my old 6850 and it was good for like 2 week and it acted funny -- I removed the driver and all was fine reinstalled the driver and acted funny .. so I thought let me put the 7850 back in and it displayed just fine with out the 14.xx driver so I installed the old 13.12 and no more issues -- so far so good back on them now for a good month

seems like when I use any of the 14.xx drivers there was some kind of issue but I go back to the old 13.12 and can run 24-7 with out issues

I tired to downgrade the driver, i removed anything related to the current one and then installed 13.9 , it is still the same :( If i get sure about that the issue is related to hardware of graphical card, i will go and buy a new one. But still i am not sure that the problem is that.
