SD Card cannot and is completely unable to format

Hi, I've been using a 16gb sdhc card for my phone, and one day it started to bug out. I tried formatting it through windows, mac, various programs, my camera, scanned it for viruses through malawarebytes (came out with nothing). It's read and write, so I can write on it. When I plug it into my windows computer, it prompts to scan and fix, and when I try to scan it, it immediately closed out the scanning program without scanning it at all. It works fine as a regular sd card on the computer but bugs out when used with my phone (nokia lumia 521). Disk utility on mac gives me "POSIX reports: The operation couldn’t be completed. Input/output error". I've tried formatting it through disk management on windows and it tells me "The format did not complete successfully". SDFormatter from sdcard.org said "Could not format the memory card. Either the drive or the memory card is not supported by this software." Please help.

Sounds like your card has punched it's ticket. When Windows does the "scan and fix" deal, it means it's having trouble reading the contents usually due to corrupt files. Have you tried to format it from your phone? Sometimes that helps. If you are able to, but are still having issues afterwards, then the card is probably dying.

Formatting on the phone isn't working. Guess i'll have to buy a new card
