Why does my PC crash every time i turn my computer on?

Problem: My Computer dies 2-5 times every time i get to my desktop and i dont know why, can anyone please help me?

PC Specs:

CPU: AMD Phenom II X3 710 2.6Ghz heka 45nm Technology

GPU: ATi Radeon HD 4850 1024MB

RAM: 6.00 Dual-Channel DDR3 @ 669Mhz

Motherboard: Gigabyte Technology Co., Ltd. GA-78LMT-USB3 (Socket M2)

HDD: 698GB Seagate ST375052 8AS SCSI Disk Device (RAID)

Does anyone out there know why this keeps happening, if so please tell me

This looks like mis-matched RAM running at the wrong speed. You should be able to better identify what hardware you are running.

The +12v reading is too low (10.152 V). This could be caused by a bad power supply or the method you are using to get that reading.

Well all that happens is this: every time the PC crashes it reboots it self for no apparent reason, then when it turns back on it shows a black screen with white writing saying words to this effect: 'Your PC shut encountered an unexpected error and shut down to protect vital data, please choose a mode to reboot in: Safe mode, safe mode with only essential drivers, safe mode with command prompt, start windows normally' and i choose start windows normally every time.
