What could cause my PSU to blow up?

I have an r9 270x TOXIC and fx 6300 @4.3 in my build. My fist cheap Power supply blew up with sparks flying from the back. I ordered a new one (xfx 550w 80+) and it just blew up again. I sent out a ticket and now waiting for a response, but i want to know what could cause it to blow up. There was no overheating involved but the two accidents did happen during gaming. I do not have a surge protector but i would think that a 70 dollar PSU would have some sort of protection against it. Could it be the overclock? Could cable management be a problem? Motherboard? Please just share some opinions or ideas.Thank you.

I read that alot of people said this same thing, but what do you mean by "in the case". Is the wire touching something it shouldn't or what?

So basically a bad cable management, i did sort of cram most of the cable behind the case, maybe something went wrong there.
