Adaptive V-Sync - Was it always like this ?

So I never before used adaptive vsync but recently I realized that there is no completely smooth experiance at 1080p60fps despite locked 60fps except some games.

For example I had weird shuttering in GTA5, Diablo 3, Dota 2 and couple more AAA titles, but after forcing Adaptive through Nvidia Panel and disabling ingame Vsync all games became smooth as hell, makes me wanna play even more now all those titles again.

So my question is, was this always since appear of Adaptive Vsync and I was missing it or depends from title to title ? I feel like that Adaptive Vsync is must have ON, but again a lot of people dont realize that.

Even in games with 59hz cap in settings, adaptive makes it on locked 60fps while with ingame vsync I get 59fps and most likely shuttering.
 
It would depend on title to title.
I have adaptive vsync on at the moment as some of my games exceed 60 fps and cause screen tears.
Well, technically, it would appear smoother because it's maintaining a consistent amount of FPS throughout. Normal vsync, as how I know, caps the fps to screen refresh rate and causes even more lag during intensive moments. Adaptive, as the name would suggest, disables vsync when the fps seem to drop too much, leading to a lot more consistency.

Maybe that is the reason why you seem to experience a much more smoother experience with it on than off.
 
If Adaptive V-sync makes a difference, that means your system is not maintaining 60 FPS. If your system maintains 60 FPS 100% of the time, the two work identically. What makes adaptive V-sync special, is that once you no longer can maintain 60 FPS (or rather 16.7ms frame times), it disables v-sync to avoid the stuttering caused by jumping between 16.7ms and 33.3ms frame times.

The downside is you get tearing. I have a 120hz monitor, which greatly reduces how noticeable the tearing is, so I just leave adaptive v-sync on for all games.

Well I'm more then sure that I can double framerate then 60 in 99% of the games so thats why I wondered in first place. It's on 60fps, no screen tearing but small shuttering here and there, while with adaptive is completely smooth, almost like 144hz monitor for a bit (from someone who dont really game on 144hz rather then just playing tournaments in csgo). Weird, it's not complicated or anything but I wanted to clarify that.

There could be a difference between the in game v-sync and Nvidia's driver v-sync. Try using normal v-sync force on from the Nvidia control panel.

The difference may also be as I mentioned before. You may be getting occasional slow frames. With V-sync, that slow frame results in a stutter, but adaptive v-sync handles those slow frames better. As you mentioned, you'd often see 59 FPS instead of 60. That is due to an occasional slow frame.
