Black Screen Crash In Multiple Games

Hi, I have a problem where some games crash with a black screen and the only thing that shows up and functions is the taskbar. It always shows that the game is still running and if i try to open task manager to close the game and get rid of the black screen, the task manager in the taskbar shows up but the black screen covers it in the actual desktop so I can't use it. In the end i'm always forced to restart my computer to restore everything to normal. I always monitor temperatures and keep everything up to date so don't bother asking. Nothing is overclocked besides the CPU, but it has been overclocked for a very long time without any issues until recently, no temperature problems either. Anyone have any ideas on how to fix this?
Games that give me this black screen crash or similar is Mortal Kombat X, The Witcher 3, and Starcraft 3

Did you upgrade the Nvidia drivers to the new ones that came out this week?
Is that also about the time that the problem started?
If you answered yes to both of the above questions, I would suggest going to System in the Control Panel, selecting Devices, Video Adapters, and rollback to the previous drivers until Nvidia can get newer drivers out.

Problem was there before this driver came out, and yes like I already said I always keep drivers up to date. I usually keep the power target of my GPU at max (124%) using EVGA Percision X. I will try setting it to default (100%) but I doubt this will change anything.
 
Next thing I would try would be to remove the video card from the motherboard. Clean it up while its out, might also want to clean out the case while the video card is out as well. Then put it back in, making sure its firmly seated in the slot. Dont forget to re-attach the power cables.

Power back up, and see if that stops it.

If the problem continues, I think I would download the MemTest86+ ISO image, burn the disc, and then boot that disc and let it test your memory and CPU for at least 10 hours. See if it makes it through that test or not.

Something is randomly failing or not connected well, and its a pain sometimes to figure out what the something is. But through various tests and procedures, we can slowly eliminate the possibilities and sooner or later the problem item will rear its ugly head for you. Or it will magically stop being a pain.

So try the above. Lets see if that changes anything. If not, we'll try some new things.

Problem is, this is an insane amount of work. Is there some software troubleshooting methods that I could try before looking at hardware? on a side not though, Just a few days ago I ran into a problem right after I exited witcher3 because I pressed the default buttom on EVGA precision X and all of the sudden, the screen went black and said "signal out of range, please revert back to default resolution" so I was forced to use my friend's monitor to try and set it back to normal resolution, which fixed it, but I accidentally pressed that default button again then I was forced to use my TV which didn't have my monitor's resolution so I tried uninstalling display driver and that's what caused a heap of trouble because the driver refused to uninstall even after using Display Driver Uninstaller software. I then had to do a system restore and then I reinstalled drivers and used DDU again and it worked. Yesterday I also got a display driver crash randomly when I was watching a Twitch video while running a scan and that's the first time in a long time that I have ever had this display driver crash error "display driver crashed and has recovered".
 
I suggested the two things I felt would most likely either fix the problem or tell you what the problem is. At the very least we should find out if your power supply is the problem or not, and if the memory is stable or not.

We have to start somewhere. If I could sit down and write a program that would run even on faulty hardware, and diagnose all of the problems even in a dead computer, and tell you what is exactly wrong with your PC, I would become a billionaire in about 2 years. Maybe less. Unfortunately for both of us though, while I am a programmer, I cannot trust anything a malfunctioning computer tells me, so I probably am not going to become a billionaire in the next few years.

What you do is your choice. I stand by my suggestions to you.

Ok, I will try what you have suggested but I was asking if we should see if this is a software issue and not a hardware issue unless you're certain that this must be a hardware issue.

ok so i tried both suggestions. Memtest returned no errors and i removed and reinstalled the card. I also completely uninstalled and reinstalled the driver using Display Driver Uninstaller, but i got a crash today "display driver crashed and has recovered" while watching Twitch. I looked up the driver (352.86) and it seems like a bunch of people are also having issues with it, but i just want to confirm that it is indeed a driver issue.
 
Control Panel -> System -> Device Manager -> Display Adapters -> Right Click on your video card -> Properties -> Driver tab -> Roll Back Driver.

Reboot after that and see what happens.

If you want to go back more than one driver version, you will need to go to the Nvidia web site and download it from there.

I THINK I tried every driver from 347.88 up until the latest Windows 10 driver (the one i'm using now). They all do the same driver crashed and has recovered thing. Sometimes it doesn't show the message and displays it in windows even logs instead and sometimes it shows the black screen with only task bar being functional, but unable to actually use task manager since it only displays task manager UNDER the black screen so it ends up forcing me to restart, but sometimes it also shows a black screen for a second or 2 then brings me right back to the game. I think percision X may be the problem, but i can't stop using it because I wanta keep the fan profile.
 
Have you ever run Display Driver Uninstall and then once that was done, reinstalled the video drivers? If not, click that icon below to download it, and when it asks to go into Safe Mode, select yes and let it restart and then pick the top choice to clean out all the old gunk the video drivers have left behind in the past.

When that is done, reinstall the latest video drivers and see if anything changes for you.
