My USB flash drive are not working properly anymore

The last time I know that I use my flash drive is I changed it into NTFS from being FAT32 so I can burn an iso file ( I forgot now what I am trying to install ) for Installation of a software. It was successful. After it, I changed it back into FAT32 , runned chckdsk and reformatted.
Months later When I am trying to move files on it and creating a folder I am getting "Error 0x8007045D: the request could not be performed because of an I/O device error"

My rig is:
Athlon II X2 250
Radeon HD 6570
4Gb 1600 (1x4Gb)
WD 320gb partitioned in to 3: (C: ) 171gb (D: ) 126Gb (F: )100Mb
Windows 7 Ult
USB flash drive Toshiba 16GB
Softwares used: Rufus and Daemon
 


Is there any chances to revive it? I didn't find any solution to revive it on google, I just wanna make sure from experts here.
Thing is, It has sentimental value to me. I really do like to use it.
