*NEW INFORMATION* After one of these freezes I alt-tabbed and caught a quick glimpse in the corner of my screen of an error saying that my video card had failed.

So basically any graphics-intensive games that I play seem to have the same/similar problems.
I am pretty sure that my graphics card is failing but I'd like a second opinion before I go out and get another one. Thanks for any help!

i think you can use a better psu to power the card more better it could be a display driver failure or check your clock speeds they might be pushed to high which fails the card and freezes the screen due to render failure

I haven't touched my clock speeds in a while so it should still be the standard speeds, and I've never had problems with my psu before.

I've used the same PSU for a few years now, but I guess it was inevitable that it couldn't keep up with my GPU; was about time for an upgrade anyway. Thanks for taking the time to help, I'll be sure your advice doesn't go to waste!

One final thing I'd like to note. When a graphics card is underpowered, or the PSU is low end and has bad voltage regulation which may actually overvolt the GPU and other components, or allow an exceptional amount of ripple or noise, this can affect hardware like the GPU and cause symptoms that would seem to be a failure of that component, but is actually just the PSU. In some cases, the PSU may also physically damage the hardware, so that's something to consider since that's definitely not the best PSU, by far, and is lower than the recommended capacity for your card even if it was a brand new high end unit.

I found it was applications/services that were starting up with windows, did a clean boot. went into MSCONFIG / services , clicked hide Microsoft services , then disabled all, then task manager/ startup and disabled everything, when you find this works you can go back and turn on a few services /apps at a time and reboot till you find the one(s) it was.
