For the past few days i have been having an issue. It all started 3 weeks after getting my gtx 970, thing all worked fine and then in the midst of playing the witcher 3 i lost all signal to my monitors permanently. I sent the card back in after i put my gtx 660 back in. The gtx 660 worked for about a day just fine and then the day after i started having my driver crash and recovery issues. This would happen occasionally in all games and most often at the end of each match during heroes of the storm is a guarantee trigger.
Now at this point it is happening very often, during videos online and even straight up crashes and restarts me computer. I am also having an odd popping sound with my audio which gives a slight pop sound after each thing of noise like click through files on your computer and after the initially windows noise a pop sound appears.

Things that i have tried:
-roll back drivers to 3 different versions
-format drive and reinstall windows
-reconnect everything in my computer also checked and nothing seems odd
-memtest and everything passed
