I am looking for a graphics card. The current one I have is causing a bottleneck and I want to be able to run most modern games (GTA V, etc) at medium 1920x1080.

My current PC specifications
GPU: GeForce GT 610 - PCI Express x 16
CPU: AMD FX(tm)-4300 Quad-Core Processor
Memory: 8GB Usable
Power supply: 350 watt
Motherboard: AMD 760G Chipset

Budget: $350

GTX 960. MSI, Asus, and Gigabit are all great brands. And you will need to upgrade to a 500W power supply. EVGA makes a good budget unit: http://www.amazon.com/EVGA-80PLUS-Certified-ATX12V-100-W1-0500-KR/dp/B00H33SFJU But Antec makes a higher quality unit but is less watts (which I cannot find a link to right now). There is also the XFX TS 550W which is very high quality, plenty of power, and still fits in your budget.

If you want some better performance you can go with the AMD 280X graphics card instead for about the same price but with that comes higher heat and power usage.

with your current PSU you can get a 750Ti which can handle GTA V on medium settings.

If you want to crank up to high-very high-ultra settings, GTX 960 will be much better but you need to have at least a good 400w-450w PSU
